Long Term Care Services
Mercy Clinic LTC Services is a medical practice that has dedicated itself to the
care of patients in the Long Term Care (LTC) setting. We strive to establish the
highest standard possible in the care of beloved elderly.
The physicians are Board Certified in either Family Practice or Internal Medicine,
as well as Certified in Geriatric Medicine. They are supported by Non-Physician
Providers (NPP) that have been trained in Geriatric Medicine and Long Term Care.
All are members of the American Medical Director Association, an organization dedicated
to the care of the aged in the long term care setting.
Our goal is to provide intensive physician or NPP presence in the long-term care
setting, to assist in resident care, provide early medical intervention, educate
nursing home staff, lower healthcare costs, and minimize hospitalization. This has
been a successful style of care in multiple communities across the country.
If a resident needs hospitalization and is sent to Mercy Medical Center, the LTC
Physicians will/can assist in the continuality of care and make appropriate referrals
as needed. Once medically stable at the hospital, transferring the resident back
to the LTC setting is enabled by the active involvement of the physicians in the
care.
