Fees

Your charge is based on the number and type of services that our clinic staff provides to you. Fees for office visits will vary, depending on the level of care you receive and the time required for evaluation and treatment. Laboratory and x-ray services are additional charges that are not included in the office visit fee.

Insurance and Payment

We accept most insurance plans. At the time you schedule your appointment, please verify that your plan is one with which we participate.

If your insurance is an HMO that requires referral by your primary care physician, please obtain your referral prior to the date of your appointment.

If your insurance is a co-payment plan, you will need to pay your co-payment at the time of service.

For plans with which we do not have a contract , we ask that you pay at the time service is provided. Our staff will help you with the information you need to file with your insurance company.

You may pay with cash, check, MasterCard or Visa.
